Important vaccination to obtain preconception
Influenza
TDaP (Tetanus, Diphtheria, Pertussis)
MMR (Measels, Mumps, Rubella)
Varicella
HPV
Hepatitis B
Influenza
It is important for everyone to be immunized, particularly during flu season. If there’s a possibility of pregnancy at the time of vaccination, an inactivated vaccine would be preferable.
Varicella
Adult recommendation is 2 doses (if born in 1980 or later)
Varicella vaccination is contraindicated during pregnancy. If a preconception patient without immunity to varicella is planning/willing to postpone pregnancy until varicella series is completed (2 doses, 4-8 weeks apart), consider recommending vaccination prior to pregnancy.
TDap
Adult recommendation is 1 dose Tdap, then Td or Tdap booster every 10 yrs.
The current recommendation is for pregnant individuals to get Tdap in the third trimester of each pregnancy. Preconception patients should be made aware of this recommendation.
MMR
Adult recommendation is 1 or 2 doses depending on indication (if born in 1957 or later).
**MMR is one of the most important vaccinations to encourage patients to get BEFORE PREGNANCY because it should be given at least one month prior to pregnancy. Rubella is contagious and can cause pregnancy loss or birth defects.
HPV
Adult recommendation (age 19-26) is 2 or 3 doses depending on age at initial vaccination, For age 27-45, shared decision making should guide recommendation.
According to the CDC, HPV vaccination is not recommended until after pregnancy; no intervention needed if vaccinated while pregnant; pregnancy testing not needed before vaccination.
Hepatitis B
Adult recommendation is 2 or 3 doses depending on vaccine for individuals with risk factors
If a preconception patient is at risk for infection or wants protection from Hepatitis B, it is ideal to complete the series prior to pregnancy. However, pregnancy is not a contraindication for Hepatitis B vaccination.
