Basic function of potassuium
Intracellular excitation. Maintains cardiac muscle function. NEEDS TO BE RIGHT IN RANGE, may be no symptoms at first, but when it does happen it is dramatic
Causes of Hypokalemia
“Body is trying to DITCH potassium”

Drugs (beta-adrenergic agonists or antagonists, diuretics, corticosteroids),
I POC
Inadequate intake of potassium ( NPO)
T POC
Too much water intake
C POC
Cushing’s syndrome (too much secretion of aldosterone)
H POC
Heavy fluid loss (NG suction (GI secretions really rich in K+), (vomiting, diarrhea, sweating)
S&S of Hypokalemia
Everything is “low and slow”
Pulse PO
weak, thready, irregular
Bowel sounds PO
Decreased (Constipation)
DTR’s PO
Decreased
Orthostatic… PO
hypotension
EKG changes PO
Depressed ST segment, flat or inverted T wave, and a prominent U wave
3 L’s PO
- Lethargic*
- Low, Shallow, Respirations* (Assess rate, depth, effort, and O2 sat.)
- Low BP and HR*
NEVER give potassium through…
IV push, subcut, or IM injection
IV potassium…
needs to be diluted and administered through an IV pump
Causes of Hyperkalemia
“Body CARED too much for potassium”

Cellular movement of potassium from the intracellular to the extracellular (burns, acidosis, other tissue damage)

Adrenal insufficiency (Addison’s Disease)
R PERC
Renal failure, RBC infusions
E PERC
Excessive potassium intake
D PERC
Drugs (potassium sparing drugs, ACE inhibitors, NSAIDs) Diabetes
S&S of Hyperkalemia (tight and contracted)
MURDER
M PER
Muscle (leg) weakness
