Postnatal period Flashcards
A process by which the uterus returns to its pre pregnant size, position and tone.
the vagina, uterine ligaments, and muscles of the pelvic floor all revert back to pre pregnant state.
Uterus weight from 1000 to 60 gm
Involution
Begins immediately following the delivery of the placenta and membranes
6 weeks
Ends with the return of fertility
Puerperium / postnatal period
What are the uterine involution process?
- Contraction of the uterine muscle fibre
- Catabolism
- Regeneration of uterine epithelium
This process is immediately after placental delivery, and helps control the bleeding.
Contraction of the uterine muscles
Process in which levels of estrogen and progesterone cause uterine cells to atrophy.
Process in which protein material in uterine wall is broken down cause of reduction in size.
Migration of macrophages causes autolysis, products of this process is absorbed by blood stream and excreted through nitrogenous waste
Process of Catabolism
Process of the outer portion of endometrial layer is expelled with placenta.
Process in which remaining decidua separate into two layers in 2-3 days. (Superficial layer sheds & basal layer remains and is the source for new endometrium.
Process of regeneration of uterine epithelium.
How many weeks does regeneration of endometrium excpet at placental site take?
2-3 weeks
How long does it take for placental site to regenerate?
.
6-7 weeks
(Red) vaginal excretion for 3 days
Lochia rubra
Pink brown excretion that contains leukocytes, mucous, vaginal epethelial cells, necrotic decidua, non pathological bacteria.
Sweetish in odour, lasts for 7 days
Lochia serosa
Yellow white excretion, 10th-14th postnatal day.
Mostly serous fluid and leukocytes
Plus cervical mucous and organisms
Lochia Alba
Possible causes for presence of clot or excess bleeding (2pads/hr)?
Possible, inadequate uterine contraction hence bleeding from vessels at placental site, laceration, hemetoma, and coagulation disorder.
Possible causes of persistent lochia rubra?
Possibly, inadequate uterine contraction, retained placental fragments, infection
Immediately after delivery, the top of the fundus is in the midline and approximately 2/3 - 3/4 of the way between symohysis pubis and the umbilicus
Involution of the uterus
6-12 hours after birth, the fundus is at the level of or one fingerbreadth below the…
Umbilicus
