What is postnatal depression?
Characterised by low mood in the postnatal period (within first 3 months)
Usual onset of postnatal depression?
1-4 weeks postpartum
Risk factors for postnatal depression?
- Family or personal history of depression or anxiety
- Complicated pregnancy
- Traumatic birth
- Relationship difficulties (including domestic violence)
- History of abuse or trauma
- Lack of support
- Financial difficulties
Clinical features of postnatal depression?
Low mood
Anhedonia (lack of pleasure in activities)
Anergia (low energy)
What is the difference between postnatal depression and “baby blues”?
“baby blues” do not last for more than 2 weeks after giving birth.
Management of mild postnatal depression cases?
Managed with additional support, self-help and GP follow-up.
Management of moderate postnatal depression cases?
Managed with antidepressant medications (e.g. SSRI’s) and CBT (cognitive behavioural therapy).
Management of severe postnatal depression cases?
May require input from specialist psychiatry services, in rare cases inpatient care on mother and baby unit may be required.
