Define
Inflammation of the posterior sclera (posterior to the insertion of the rectus muscles)
Etiology
idiopathic
Collagen vascular disease
Systemic infection
Symptoms
Minimal to no eye redness Severe, boring eye pain. -Awakens patient -May radiate -Worsens with eye movement -Tearing
Blurry vision
Signs
Need to do a dilated exam!
Chorioretinal folds that run horizontally. Not as much tautness to the retina. Like a rug folded up
Proptosis
Restricted motility
Hyperopia (because globe is pushed forward)
Peripapillary T sign on B scan. (fluid between Tenon’s capsule and sclera creates a squaring off of the interface between the ON and sclera)
Complications
Choriodal detachment
Retinal pathology
Optic disc edema
Glaucoma secondary to increased IOP
management
Oral NSAIDs. Prob refer. Order lab work.
Can posterior scleritis ever be an extension of anterior scleritis?
