Orbital cavernous hemangioma- what is it?
A collection of small blood vessels surrounded by a fibrous pseudo capsule.
*Large vascular spaces are within the mass, thus the term cavernous.

Proptosis, globe displacement, restricted EOMs
Complications
Compression on the globe and/or ON
Thrombosis
Management
Orbital CT or MRI
If symptomatic or complications, refer out for complete surgical excision.

Abnormally enlarged vein that becomes dilated with increased venous pressure- during valsalva maneuvers, bending forward, or holding breath.

Intermittent bulging eye/displaed eye.
Intermittent double vision.

INTERMITTENT proptosis

Pt complaining of double vision and proptosis that comes and goes

Compression on the globe or ON
Orbital hemorrhage - rapid and painful proptosis
Thrombosis

Orbital CT or MRI
If complications, refer out for surgery

Lacrimal gland pleomorphic adenoma (benign mixed cell tumor)
What is it?
Benign tumor of the lacrimal gland due to proliferation of epithelial cells
Ages 30-40 years old.
Symptoms of Lacrimal gland pleomorphic adenoma (benign mixed cell tumor)
Displaced eye, double vision, eyelid swelling and redness.
Signs of Lacrimal gland pleomorphic adenoma (benign mixed cell tumor)
Globe displaced inferior and medially
Restricted EOM
Eyelid edema and redness
Palpable lacrimal gland mass
Lacrimal gland pleomorphic adenoma (benign mixed cell tumor) complications
Malignant transformation
Lacrimal gland pleomorphic adenoma (benign mixed cell tumor) management
Orbital CT or MRI
Refer out for complete surgical excision since it could become malignant
progression of Lacrimal gland pleomorphic adenoma (benign mixed cell tumor)
Slow, painless progression over the course of 12 months.
**rapid progression and pain may indicate malignancy.
