Symptoms

A

Red eyes and EXTREME pain.

Awakens patient at night, radiates to other parts of face, worsens during eye movement, may have tearing.

Signs

A

Anterior scleritis progresses to isolated patches of scleral edema with overlying non perfused episclera and conj. Appears white!! This is a big red flag. white, pearly, opaque.

They can rapidly coalesce and proceed to progressive scleral necrosis. This appears blue-grey.

Complications

A

Inflammation of adjacent K (peripheral keratitis)
Glaucoma
Scleral perforation

management

A

Prob better to get help with this. Refer.

Non infections:
Oral steroids
IV steroids
Oral immunosuppressive
Recommend glasses or eye shield for protection.
Surgery possibly if scleral perforation occurs (scleral patch graft)
