DDx for fever post-op (8)
A
- malignant hyper
- atelectatis - most common
- bacteremia - within 45 minutes
- pneumonia - if atelectatis not resolved
- UTI - 3 days
- DVT - POD 5
- wound infection - POD 7
- abscess - POD 10-15

DDx for chest pain (2)
A
- peri-op MI - during or first few days post-op
2. PE - sudden pleuritic with SOB

A
- aspiration
2. intra-op tension pneumo

5 DDx for disorientation/coma
A
- hypoxia - ABGs
- ARDS - pulmonary infltrates with no evidence of CHF
- DTs
- hyponatremia - ADH or wrong fluids
- hypernatremia - look for unreplaced urine outputs

3 possible urinary issues
A
- retention - may need in/outs
- zero output - look for block
- low output - either fluid deficit or AKI
- can test with a bolus
- or look at urine Na - if low, kidneys likely okay

A
- illeus
- early mech obstruction
- ogilvie (illeus of colon)

A
- dehiscence
- evisceration
- infecitons
- fistulas of GI

6 common lyte issues
A
- hypernatremia - lost water and can fix with slow d5w
- hyponatremia - ADH or inappropriate fluids
- hypokalemia
- slow if lost through GI or renal (lasix)
- fast through over correction in ketoacidosis - hyperkalemia
- can’t renally excrete
- crushed cells - met. acidosis
- extra acids - lactate, keto
- loss of buffer
- renal inability to excrete acid - met. alkalosis
- loss of gastric juices
- too much bicarb - resp acidosis or alk - ventilation issues
