esophageal atresia
associations with atresia
VACTER
- vertebral anomalies
- anal imperf
- Cardiac
- tracheal
- esophagela
- renal/radial
imperforate anus
need to rule out other VACTER associations
- looks for nearby fistula
- if not need to do colostomy
congenital diaphragmatic hernia
always on left
- need to wait for 3-4 days to repair
gastroschisis and omphacoel
abdo contents coming out of the bowel wall
- need to slowly be put back in
- eventually can close defect
extrophy of bladder
need to be repaired right away
what is green vomiting
bad sign
green vomiting and double bubble
duodenal atresia, annular pnacreas, or malrotation
intestinal atresia
green vomit and multiple small air-fluid levels
- vascular accident in utero
signs of necrotizing entercolitis
seen when first fed
- feeding intolerence, abdo distension, rapid drop in platelets
- stop all feeds, IV ABX and fluids
meconium ileus
CF babies (look for hint in mother)
- dilated loops of bowel
- gastrografin is diagnostic and therapeutic
hypertrophic pyloric stenosis
nonbilious projectile vomits after feeds
- hungry and dehydrated
biliary atresia
6-8 week old babies that turn yellow
- serologies and sweat test to rule out other issues
- phenobarbitol stim and if none reaches the duodenum, then there is an issue
intussuseption
chubby 6-12 month olds
- bouts of coliky abdo pain and get up and squat to resolve
- then comes back
- vague mass on R side and currant jelly stools
- Dx and Tx with barium or air enema
meckels
should be suspected when there is a lower GI bleed in young ones
