What is positive identification?
documenting who did what
There must be positive identification of the pharmacist or pharmacist(s) responsible for performing all activities relating to the practice of pharmacy including, but not limited to:

d) Patient counseling
e) Administering adult immunizations
f) Prescription information reduced to writing from an order received by
telephone, facsimile, or recording device.

i) Any changes made to a prescription or medication order
How many years do invoices for controlled substances need to be kept?
3 years (Keep invoices of III’s, IV’s, V’s in one file and invoices of II’s in another file.)
How many years to get rid of outdated drugs?
1 year at pharmacy level
How many years to get rid of outdated drugs at wholesaler level?
2 years
What needs to be included on prescription label?
- name, address, phone number of dispenser
- Name of patient***
- a compound must have “Compounded Drug Product” on label or bottle
What is required for prescription label of an animal?
- last name of owner
- name of animal
- species of the animal
Ex/ Sullivan, Albie, rabbit
What does the term affix mean?
The term “affix” means the prescription label must be attached or fastened to the drug’s container.
Who is licensed to prescribe?
a) MD, DO, Podiatrist, Vet, DDS,
b) Nurse practitioner (APN)
c) Optometrist (prescribing certificate and for conditions related to the eye)
d) Interns and residents
e) Physicians assistants
f) Out-of-state practitioners
g) Limited practitioners
Who are the limited practioners that can only write prescriptions within the scope of their practice?
veterinarians, optometrists, dentists, podiatrists
What is a R. Ph. (Responsible Pharmacist)?
R.Ph. who signs the terminal distributor’s license
Can a R. Ph be a responsible for 2 pharmacies?
Can only be responsible pharmacist for one pharmacy or campus (special permission can be granted for more than one)
For a R.Ph, how many days to notify the board of the change?
10 days Change in Responsible Pharmacist (Whenever there is a change is responsible pharmacist, the Board of Pharmacy must be notified by the new responsible pharmacist within 10 days of the change by a Board approved method)
What do new R.Ph have to do?
a complete inventory must be taken of all controlled substances
