Polymyositis and Dermatomyositis Flashcards
What do polymyositis and dermatomyositis have in common?
Autoimmune disorders where there is inflammation in the muscles.
What is the difference between polymyositis and dermatomyositis?
- Polymyositis - chronic muscle inflammation
2. Dermatomyositis - connective tissue disorder where the skin is also involved
What is the key investigation for diagnosing myositis?
Creatine kinase - over 1000 U/L in myositis
What are the main causes of a raised creatine kinase?
- Myositis
- Rhabdomyolysis
- AKI
- MI
- Statins
- Strenuous exercise
Which cancers are commonly associated with myositis?
Lung, breast, ovarian, gastric.
What is this a presentation of?
Muscle pain, fatigue, and weakness occurring bilaterally and affecting proximal muscles, developed over weeks.
Polymyositis
What are the skin features found in dermatomyositis?
- Gottron lesions on knuckles, elbows, knees
- Photosensitive erythematous rash on back, shoulders, and neck
- Purple rash on face and eyelids
- Periorbital oedema
- Subcutaneous calcinosis
What are Gottron lesions and what condition are they found in?
- Scaly erythematous patches
2. Dermatomyositis
Which conditions are Anti-Jo antibodies found in?
Polymyositis and dermatomyositis
What condition are Anti-Mi-2 and Anti-nuclear antibodies found in?
Dermatomyositis
How are polymyositis and dermatomyositis diagnosed?
Clinical presentation, creatine kinase, autoantibodies, electromyography, muscle biopsy for definitive diagnosis.
What are the treatment options for polymyositis and dermatomyositis?
- Corticosteroids 1st line for both
2. Immunosuppressants (azathioprine), IV immunoglobulins, biological therapy (infliximab/etanercept)
