What is ‘polymyalgia rheumatica’
A
common inflammatory rheumatic disease which affcts elderly folk

How does polymyalgia rheumatica classically present?
A
- new onset pain in shoulder, hip or neck
- systemic feature - fatigue/fever/wt loss
- morning stiffness

Which joints are most affected
A
- shoulders
- hip
- neck

features of polymyalgia rheumatica in the joint
A
- symmetrical
- acute pain lasts > 45 minutes
- worse in the morning
- without weakness !

what are assoc systemic symptoms?
A
- weight loss
- fever
- fatigue
- depressed mood

which condition is polymyalgia rheumatica associated with?
A
Giant cell artertitis - 20% with polymyalgia rheumatica progress

What are the results of lab tests?
A
- very high ESR and high CRP
- normal creatinine kinase and no auto antibodies
- normocytic anaemia

What does the ultrasound show
A
bursitis

Mx of polymyalgia rheumatica
A
low dose glucocorticoid - 15 mg of predisolone and taper
