What is a pleural effusion?
A build up of fluid in the pleural space of the lungs.
What are the types of pleural effusion?
Exudate - capillary permeability
Transudate - interstitial fluid
What is the protein count of an exudate pleural effusion?
> 35g/L

<35g/L.
What are the main causes of exudative pleural effusion?
INFLAMMATION: pneumonia lung cancer tuberculosis rheumatoid arthritis
What are the main causes of transudate pleural effusions?
congestive heart failure
hypoalbuminia
hypothyroidism
What are the symptoms of pleural effusion?
SOB
Reduced exercise tolerance
chest pain
what are the signs of a pleural effusion?
If large - deviation of mediastinum/trachea
reduced breath sounds
dullness on percussion
reduced chest expansion on affected side
what are the investigations done for pleural effusions?
X-ray
Aspiration
what are the signs of a pleural effusion on a chest x-ray?
Blunting of costophrenic angle
fluid in lung fissures
meniscus (in large effusions)
tracheal and mediastinal shift
What is shown in aspiration?
pH
White cell count
Protein count
What is an empyema?
A pus/infection which is found in the pleural space.

ACIDIC <7.2
pus
low glucose
High LDH

what is the typical history of one empyema?
Improving pneumonia but new/ongoing fever.
