A: Paranoid, Schizoid, Schizotypal. B: Antisocial, Histrionic, Narcissistic, Borderline. C: Avoidant, Dependent, Obsessive-Compulsive
“Distrustful, Suspicious, Litigious, Attributes responsibility for own problems to others, Interprets motives of others as malevolent. Blames physician for illness, Is overly sensitive to a perceived lack of attention from physician.”
Paranoid PD
“Long-standing pattern of voluntary social withdrawal w/o psychosis, Detachment form others, Restricted emotions, Little emotional connection with physician, Withdrawn when ill.”
Schizoid PD
“Peculiar appearance, Magical thinking, Odd though patterns and behavior without psychosis. Behaves strangely and even inappropriately when ill.”
Schizotypal PD
“Attention-seeking, Extroverted, Emotional, Sexually provocative, Shallow/Vain, Unable to maintain intimate relationships. Dramatic reporting of symptoms, Approaches physician in a sexual or other inappropriate manner during illness.”
Histrionic PD
“Pompous, Sense of special entitlement and lacks empathy for others, Feels superior to others. Has perfect self-image that is threatened by illness, Often demanding when ill, Avoids even necessary treatment if it affects physical appearance.”
Narcissistic PD
“Refuses to conform to social norms, Dishonest, Associated with conduct disorder in childhood (before age 15) and criminality in adulthood, ‘Psychopath/Sociopath.’ Has no concern for others, Fails to learn from experience.”
Antisocial PD
“Erratic, Unstable behavior and mood, Boredom, Feelings of alonenes, Impulsiveness, Suicide attempts, Self-injury, Often combined with mood and eating disorders. May show ‘minipsychotic’ episodes, Overidealizes and then overreacts to perceived rejection by physician.”
Borderline PD
“Timid, Overly sensitive to criticism/rejection, Socially withdrawn, Feelings of inferiority. Fears rejection by physician, Avoids test/treatment.”
Avoidant PD
“Perfectionistic, Orderly, Stubborn, Indecisive, Ultimately inefficient. Fears loss of control and may try to control dovotr during illness, Follows doctor’s orders to the letter.”
Obsessive-Compulsive PD
“Poor self-confidence, Allows others to make their decisions and assume their responsibilities, May end up as an abused spouse. Has an excessive need to be cared for by others, Shows helplessness and the desire for attention from the physician during illness.”
Dependent PD
“Procrastinates, Ineffecient, Sullen, Outwardly compliant but inwardly defiant. Asks for help but then does not adhere to the physician’s advice.”
Passive-Aggressive
Tx based on cluster?
A: Maybe antipsychotics, psychotherapy. B: Mood stabilizers, antipsychotics, psychotherapy. C: Psychopharmacology, psychotherapy.
