Spontaneous bacterial peritonitis

In who is SBP most commonly seen in?
A
Pts. with:
Cirrhosis
End-stage liver disease

What are the symptoms of SBP?
A
abdominal pain, fever, vomiting, altered mental status, and gastrointestinal bleeding
BUT
Pts. commonly minimally or asympto

How is SBP diagnosed?
A
Paracentesis: neutrophil count > 250 cells/ul
Presence of E. coli in fluid culture

What is paracentesis?
A
procedure that removes fluid (peritoneal fluid) from the abdomen through a slender needle

What is the management of SBP?
A
IV Cefotaxime

When should AB prophylaxis be given?
A
Pts. with ascites who
- have had an epsiode of SBP
- fluid protein <15 g/l and either Child-Pugh score of at least 9 or hepatorenal syndrome

What can cause peritonitis?
A
Burst stomach ulcer Burst appendix Crohn's or diverticulitis Pancreatitis Cirrhosis Infection as a result of peritoneal dialysis equipment
