Peritonitis Flashcards
Primary peritonitis eg
feline infectious peritonitis
mechanical causes of aseptic peritonitis
- swabs
- air
- suture material
- glove powder
chemical causes of aseptic peritonitis
- uroperitoneum
- pancreatitis
what causes septic peritonitis?
any source of intra-abdominal bacterial contamination
radiographic signs of peritonitis
- generalised loss of serosal detail
- focal loss of detail
- free gas developing around the diaphragm/abdomen
abdominocentesis technique
- blind/US guided
- aseptic technique
- needle placed 1cm caudal to umbilicus
technique for diagnostic peritoneal lavage
- Intraperitoneal infusion w/ saline 20ml/kg
- Roll animal
- Retrieve fluid back into a saline bag - only need a small amount
Normal cell count and TP of abdominal fluid
cells <300/uL
protein <3g/dL
septic indicators on abdominocentesis biochem
low glucose and high lactate (vs. serum levels)
uroperitoneum indicators on abdominocentesis biochem
creatinine 2.5x serum
potassium 1.4x serum
indicators of chyloperitoneum on abdominocentesis biochem
triglycerides 3x serum
surgical tx. of peritonitis
- ID and correction of the underlying cause
- Lavage the peritoneal cavity (local vs. entire abdomen)
- Consideration of post-op drainage
- Provision of an avenue for nutritional stupport
post-op management
- ABs: continue empirical until culture return
- Analgesia: reduce sympathetic stim. that causes ileus
- IVFT
- Nutritional support: reverse catabolic state, protein replacement
