What causes PVD?
Usually atherosclerosis
What is critical limb ischaemia?
End stage PVD
What are the 6 P’s of critical limb ischaemia?
Pain, Paralysis, Pulseless, Perishingly cold, Pallor, Paraesthesia
What are risk factors for PVD?
Smoking, DM, HTN, sedentary lifestyle, hyperlipidaemia, history of CAD, age
How does PVD present?
Pain in lower limbs on exercise, relieved on rest due to intermittent claudication, leg may be pale, cold, loss of hair, skin changes
How does severe PVD present?
Unremitting pain in foot, especially at night so hangs foot out of bed
How do you check for PVD?
Ankle brachial pressure index 0.90 or smaller
How do you treat PVD?
Control risk factors, antiplatelet therapy
How do you control risk factors?
Smoking cessation, regular exercise, weight reduction, BP control, DM control, statin
What antiplatelet therapy is used?
Aspirin or clopidogrel
What else do you need to do in critical limb ischaemia/
Revascularisation or amputation if unsuitable
What is the first line imaging?
Colour duplex US
