What is bundle branch block?
A block in the conduction of one of the bundle branches, so the ventricles don’t receive impulses at the same time
What does right bundle branch block look like on an ECG?
MaRRoW (R wave in V1, Slurred S wave in V6), Wide QRS
What does left bundle branch block look like on an ECG?
WiLLiaM (Slurred S wave in V1, R wave in V6), Wide QRS, notched top, T wave inversion in lateral leads
What can cause RBBB?
PE, IHD, atrial ventricular septal defect
How does RBBB present?
Asymptomatic, syncope/presyncope
How do you manage RBBB?
Pacemaker, cardiac resynchronisation therapy, reduce blood pressure
What can cause LBBB?
IHD, aortic valve disease
How does LBBB present?
Asymptomatic, syncope/presyncope
How do you manage LBBB?
Pacemaker, cardiac resynchronisation therapy, reduce blood pressure
What is heart block?
A block at any level of the conduction system in which conduction seizes
What does 1st degree heart block look like on an ECG?
PR interval >200ms
How does 1st degree heart block present?
Asymptomatic
What does 2nd degree heart block Mobitz I look like on an ECG?
Progressive lengthening of PR interval, one non-conducted P wave, next PR interval is shorter
How does 2nd degree heart block Mobitz I present?
Light headedness, dizziness, syncope
What does 2nd degree heart block Mobitz II look like on an ECG?
Constant PR, occasional non-conducted P waves, wide QRS
