Q

What are some tips for success for peripheral and CVAD blood draw?

A
  1. Tourniquets need to be tight
  2. Phlebotomy is an art- it takes practice
  3. Phlebotomy is more about FEEL than sight
  4. Hot packs can help make veins more accessible
  5. Position the patient in a fashion that makes for easier access
  6. Anchor the vein before accessing

Q

What are some IV considerations?

A
  1. Only collect blood from an arm with continuous infusion IF there is no other options
  2. STOP the infusion prior to collection for 2-3 mins
  3. Infusions with heparin need to be stopped for 10 mins prior to blood collection

Q

CVAD blood draw considerations

A
  1. Stop any infusions beforehand
  2. Aspirate to verify blood return
  3. Flush with 9mL
  4. Waste 3-5 mL and discard
  5. Collect sample
  6. Flush with 19mL

Q

How do you prevent hemolysis?

A
  1. Problem free venipuncture
  2. Use a larger bore need to draw – 21, 22, 23 gauge
  3. Use larger needles for transferring blood into test tubes – 18g blunt fill works well
  4. Tilt blood tubes so that the blood washes down the side of the tube
  5. Aspirate slowly when collecting from veins or catheters

Q

What should we know about blood culture collections?

A
  1. priority to minimize risk of contamination
  2. Blood cultures when ordered, should be collected FIRST
  3. 5-10mL of blood should be collected for each sample
  4. Never attach blood culture bottles directly to vacu-holders because beads in the culture bottle can be introduced int the patients vein

Q

Trouble shooting peripheral and CVAD blood draw?

A

No blood or an incomplete collection…
1. change the position of the needle
2. Tourniquet too tight– could obstruct blood flow
3. Change tubes- vacuum could be a problem
4. Re-anchor the vein- some veins roll
5. Have patient make a fist to help engorge muscles
6. Pre-warm area of venipuncture to increase blood flow
7. Re-hydrate patient
