periph nerve entrapments Flashcards
radial entrapment cause/tx
cause: trauma, lead poison, crutches
tx: adjust, incr strength, stretch
radial entrapment s/sx
site: spiral groove
- wrist drop, crutch palsy, erb’s palsy
- no tricep reflex, dec sens to post arm
carpal tunnel cause/tx
median n
cause: trauma, obesity, fluid retention, preg, hypothyroid, RA
tx: adjust, cock up splint, diuretic, B6
s/sx carpal tunnel
- nocturnal pn/paresthesia of hand, thenar atrophy, weak opponens pollicis (ape hand appearance), and affects first 3 fingers
- test with tinels, phalen, thumb abduct strength, dynamom grip
pronator teres syndrome cause/tx/sx
cause: bt pronator teres heads at elbow, hypertonicity dt occupation (carpenter eg)
tx: TrP, spray/stretch
presents: pn on volar aspect of forearm (ddx from CT), paresthesia in med distrib of hand, pain with pronation, thenar atrophy
median disorder
- carpal tunnel (flx retinaculum)
- pronator teres: bt pronator teres heads at elbow
- ape hand
ulnar motor/sensory
- abd digit 5, add thumb
- distal ulnar hand, digit 5
ulnar entrapment cause/tx
cause: direct or repetitive trauma
tx: adj, tape, support
ulnar n entrapment
tunnel of guyon, cubital tunnel, claw hand
dorsal scap disorder
flared scap
long thoracic disorder
WINGED scap
lateral femoral cutaneous cause, tx
cause: external pressure, obesity, tight jeans, trucker
tx: advice with weight loss, loose clothing
lateral femoral cutaneous s/sx
- burning dysesthesia on anterolateral thigh
- L2, L3, nerve roots
lat fem cutaneous disorder
meralgia paresthetica, over pubic brim under the inguinal ligament
sciatic nerve entrapment site, s/sx
- sciatica, pelvis, hip, politeal fossa
- sens pn in post butt, thigh, leg to floor (stops at knee ddx facet)
- weak knee flx and dec achilles reflex
sciatic nerve cause
trauma, traction, wallet pressure, piriformis, muscle spasm
obturator motor/sensory
- hip add
- medial thigh
femoral n. motor/sensory
- hip flx/knee ext
- anteromedial thigh/leg
femoral n. muscles
iliopsoas, quads
peroneal disorder
fibular head fx
TIBIAL N. –> medial plantar n disorder
tarsal tunnel syndrome, test with tinels at ankle
due to post traumatic ankle deform, sublux, arthritis, edema
tibial nerve entrap s/sx
- burning paresthesia on soles, aggravated by walking/standing
- dec sens on soles, weak foot muscles
tibial nerve entrap/tarsal tunnel tx
adjust, orthotic to keep foot in slight inversion
morton’s neuroma site/sx/sx
- bt MT 3-4 found with morton’s squeeze
- pn in toes, dorsum of foot
