Pulmonary Function Test (PFT)
Required when a patient’s lung volume is in question
-Pt will exhale & inhale deeply
-Refrain from smoking & using nebulizers or bronchodilators for 6 hrs
-Position:seated
-Supplies:Spirometer
Repeat test until three acceptable tests have been performed
Perform visual acuity tests
Snellen chart: (Common) several positions may be used if pt cannot read from letter E, pt cannot read the chart before beginning, position pt 20 ft from chart, line right before should be recorded
Acuity test to screen near: room must be well lit, must be 14-16 in far
Perform suture removal
-With foreceps, graphic knot on the first suture to be removed but do not pull the suture. Cut suture close to skin, not cutting skin, gently pull through skin and place on gauze sponge
Perform basic wound care
Wound may be covered with a bandage, steri-strips or gauze and tape depending on the physicians instructions.
-keep edges clean and dry or put excessive strain or pressure
Urine tests
For every test label the specimen in front of pt
Urine tests: routine specimens are most common, midstream (Pt must urinate a little, collect specimen, then empty bladder in toilet), clean-catch urine specimen requires urethral opening cleaned by pt, first of morning, timed, and fasting.
Sputum testing
Sputum test requires a deep cough from pt. A minimum of 3ml to 5ml. Must be in room temperature for immediately processing.
Saliva tests
Often used to monitor hormone, drug and alcohol levels
Collet in sterile container
Hormone test requires specimen to be frozen when sent to laboratory for testing
Fecal (poop) tests
Perform to detect parasites, blood and bacteria
Collect in clean, sealable container and send to laboratory immediately
Some tests may be collected by pt and be taken to clinical or mail for testing
Semen testing
Performed for fertility testing and collected for future insemination
Collected in sterile container, protected from light, and kept at body temperature
Must be tested within hour
Demonstrating correct body mechanisms
- Do not over reach items
- Bend at knees, not at back
- Keep weight as close to body
- Move feet on direction of the lift
- Pushing is referred to pulling if possible
- Keep knees bent and back relaxed when moving patient
- When moving pt, keep close to body and support on weak side
Perform eye irrigation
-Reasons: inflammation,drainage, chemicals, or foreign bodies
Must be perform in sterile manner to avoid risk of infection
Position: sitting or supine position with head tilted back slightly
-Gather: container with irrigation fluids, kidney basin, sterile gauze, tissues, and gloves,
-Gently open eye with firs finger and thumb of one hand
-Aim solution into lower conjunctiva sac beginning the flow at inner canthus of eye to the other
-Advice patient to not touch eye
Perform ear irrigation
- moments when ears may be impacted with cerumen and may need irrigation
- Position:Seated
- Gather: irrigation solution, basin, syringe or ear irrigation device, gauze sponge, otoscope, drainage basin, drape, cotton swabs, and gloves
- Solution should be kept at body temperature for comfort
- Gently pull ear up and back for adults, patient under 3 pull down and back
- Place tip of syringe into the ear canal and gently direct the flow towards the roof of ear canal
Perform first aid, CPR, and rapid response procedures appropriately
- If unconscious first determine if patient has pulse and is breathing
- If pt is chocking do not provide assistance until pt indicates that he cannot breath or unable to breath
- If able to breath allow cough unassisted until the foreign object is dislodged
- If appearance is gray, ashen, or blue and skin feels cool pt may be in shock
- Place pt in supine position
- syncope: If appears flushed, heavily sweating, reports dizziness, may faint, place pt in supine, loosen clothing, maintain open airway
- Ensure pt does not leave clinic for 10 minutes
Sterilization
- Sterilization helps prevent contamination
- Some items may be sterilized in an auto clave or by dry heat depending on items
- Temp for sterilization range from 250 to 320 degrees depending item
Disinfection
Disinfection can be hard to maintain (difficult to verify)
- Disinfect examination rooms
- disinfectant kill microbes
- Use: bleach and water (1:10)
