What are the common places to find a peptic ulcer?

- Duodenal cap
- Lesser curvature of stomach

Define peptic ulcer disease.
A
A break in superficial epithelial cells penetrating down to the muscularis mucosa layer.

What are the causes of peptic ulcer disease?
A
- Use of NSAIDs
- H. pylori infection

What are the symptoms of PUD?
A
- Recurrent epigastric pain (May subside with antacids)
- Nausea/ vomiting
- Haematemesis
- Persistent severe pain suggests penetration of ulcer into other organs
- Back pain suggests penetrating posterior ulcer

Symptoms specific to a duodenal ulcer
A
Pain is worse at night and when hungry. Relieved on eating.

Symptoms specific to a gastric ulcer.
A
Weight loss and anorexia.

What investigations would you carry out?
A
- Investigate H. pylori infection
- Endoscopy to exclude cancer
