what is peptic ulcer disease?
an ulceration in the mucosal wall of the stomach, the ulcer name can be dependent on where it is in the stomach
what are some common factors/cause for ulcers?
NSAIDS, alcohol, gastritis, family history, h.pylori infection
what is a major sign and symptom of peptic ulcer?
gnawing, sharp pain in or to the left of the midepigastric region occurs after eating 1-2hours after a meal
and hematemesis for gastric
melena for duodenal
pain
what is the treatment of a peptic ulcer?
medications
administer of medication like
H2 receptors
Proton pump inhibitor
antacids
mucosal barrier
antibiotics
what is the function of a ppi?
aid with decreasing gastric acid
H2 receptor and proton pump inhibitor aid with decreasing the gastric acid
Anticholinergic is prescribed to reduce gastric motility
Mucosal barrier protectants as prescribed 1 hour before each meal
H.pyrlori for antibiotics
Client education
- avoid alcohol & substances
- avoid smoking & NSAIDs
- obtain adequate rest & easy in the food
Interventions during active bleeding
- monitor vital signs closely
- assess for hemorrhage / shock
- monitor H&H
- administer blood transfusion if needed
Surgery can be an option but watch out for dumping syndrome
- the rapid emptying of the gastric content into the small intestine that occurs following a gastric resection
- symptoms are 30mins after eating
- vomiting
- borborygmi ( loud gurgling sounds )
Education ^
Avoid big meals
Avoid alcohol and caffeine
Lie down after 20-30mins to prevent emptying
