What are the 5 penicillins?
pen G/V, cloxacillin, Ampicillin/amox, Amox Clav, Pip Tazo
Describe the spectrum of Activity for PEN G/V FOR:
- GRAM POSITIVES
- GRAM NEGS
- ANAEROBES
Gram Pos:
- strep
- some enterococci- E. faecalis mostly.
- NO STAPH
Gram NEg: N. meningitidis, syphilis only.
Describe the spectrum of Activity for AMPICILLIN/AMOXICILLIN FOR:
- GRAM POSITIVES
- GRAM NEGS
- ANAEROBES
Gram Pos:
● Streptococci
● Some enterococci (E. faecalis mostly)
No staphylococci
● Listeria monocytogenes
Some:
o Haemophilus influenzae
o Escherichia coli
o Proteus mirabilis
NOTE: WE DO NOT TREAT GRAM NEGS EMPIRICALLY WIHT AMOXICILLIN CUZ OF INCR RESISTANCE. that’s what clav is for.
PLUS
● N. meningitidis
● Borreliella burgdorferi
{B HEP }
Describe the spectrum of Activity for AMOX CLAV FOR:
- GRAM POSITIVES
- GRAM NEGS
- ANAEROBES
Gram Pos:
As with amp/amox, plus:
o Staphylococci (MSSA)
Enhanced activity against:
● H. influenzae
● Proteus
● E. coli
● Klebsiella
PLUS
● Moraxella catarrhalis
● N. meningitidis
● B. burgdorferi
Anerobic coverage : Yes (g- anaerobes)

Describe the spectrum of Activity for PIP TAZO FOR:
- GRAM POSITIVES
- GRAM NEGS
- ANAEROBES
Gram pos: similar to amox clav
Gram neg:
● Pseudomonas aeruginosa
● H. influenzae
● M. catarrhalis
Enhanced activity against:
● Enterobacterales {PEK}
** PIP TAZO IS BRAODEST SPECTRUM PENICILLIN.
DO ALL PENICILLINS COVER N. MENINGITIDIS?
YES!!!
Are the penicillins great against staph aureus?
no! Cephs are better (hence use in SSTIs).
Which two penicillins cover B. Fragilis?
amox clav and pip tazo only
Do all the penicillins cover oral anaeraobes?
yes!
Do any penicillisn cover C. diff?
no!!!
WHICH agent is the broadest spectrum penicillin?
pip tazo!!
do most penicillins need to be renally ajdusted?
yes!
