Gorillacillin cuz of how broad and strong it is.

the CARBAPENEMS!!
Describe briefly the coverage of IMIPENEM. What are the four main organisms IMIPENEM DOES NOT COVER?
- does imipenem cover psueomonas?
- does imipenem cover MOST GUT ANAEROBES?
- covers MOST gram positives except MRSA.
- covers E faecalis but NOT E faecium
- covers MOST GRAM NEGS INCLUDING PSEUDOMONAS, esp if resistant to pip tax and ceftaz.
- covers MOST ANAEROBES incld gut anaerobes except c.diff.
DOES NOT COVER: MRSA, E. Faecium, C. diff, atypicals (cuz lack cell wall and carbapenems are B lactams), trep pallidum, legionella
Does imipenem need to be dose adjusted fro renal impairment?
yes!
Describe the spectrum of activity of meropenem and doripenem
- which carbapenem is best for pseudomonas?
similar to IMIPENEM
EXCEPT: Dori is best for pseudomonas, then mero, then imi.
What is the main diff btwn meropenem and imipenem in terms of coverage?
Mero= better gram NEG coverage.
IMI: better gram POS coverage.
{think: IMABONG always bright outfits and POSITIVE. MERO sounds like NERO- negative, evil emperor}
How does the spectrum of ERTAPENEM differ from mero and doripenem?
similar to mero/dori BUT doesnt cover : Pseudomonas and Acinetobacter.
**ONLY carbapanem to not cover pseudomonas.
Waht kind of dosing is ertapenem?
ONCE DAILY!! occasionally used as outpatient for this reason.
Which carbapenem has the biggest risk of seizures?
IMIPENEM.
What is the most sig ddi with carbapanems?
Carbapenemss reduce VALPROIC ACID LEVELS***
