What is pemphigus vulgaris?
Autoimmune blistering condition of epidermis
What are antibodies against in pemphigus?
Desmosomes of epidermis
What is the presentation of pemphigus>
Lesions and blisters on erythematous base
Easily rupture- bUSt
Painful but not itchy
Systemically unwell
What are the common areas for pemphigus?
Mucous membranes
Typically starts in mouth and spreads to involve skin
What is the management of pemphigus?
Systemic steroids
Immunosuppression
Supportive
What is the prognosis of pemphigus?
Mortality ~15%
Persists for life
What is pemphigoid?
Autoimmune condition affecting deeper into the skin and typically seen in older people
What are antibodies against in pemphigoid?
Dermo-epidermal junction
What is the presentation of pemphigoid?
Tense blisters that do not easily rupture
Intact epidermis forming roof of blister
Itchy and painful
Systemically well
What are the common locations for pemphigoid?
Abdo and back
Arms and legs
NO mucous membranes
What is the management of pemphigoid?
Steroids- topical or systemic
Immunosuppression
What is the prognosis of pemphigoid?
Complete remission at 1 year common
