- narrowed upper airway
- MC: Croup
Stridor XR findings
- Croup= steeple sign
- Epiglottis: thumbprint sign
- Retroperitoneal abscess: widening of prevertebral tissues
Neonatal respiratory distress
- MCC: Transient Tachypnea
- Causes: poor alveolar fluid clearance s/p C-section
Croup
- MCC of stridor d/t parainfluenza virus
- Barking cough, worse w/ irritation crying or laying down
- Steeple sign on XR
Croup management
Racemic epi and dexamethasone
Epiglottis
- MCC HIB (Haemophilus influenzae type b)
- Drooling, tripoding
- Thumbprint sign on CXR
Bronchiectasis
- irreversible dilation and destruction of bronchi
- MCC pseudomonas infection in cystic fibrosis
- foul-smelling sputum
Bronchiectasis diagnostic findings
- CT Tram-Track appearance
- Signet Sign-looks like a diamond ring (pulmonary artery coupled with dilated bronchus)
Bronchiolitis acute
MCC: RSV
Bronchiolitis Obliterans
- CXR cuffing, CT mosaic
- tx with corticosteroids and immunosuppressants
COP (cryptogenic organizing pneumonia)
- Fibrotic scarring
- Mimics pneumonia, no response to antibiotics
- tx with corticosteroids.
Cystic Fibrosis
- Autosomal recessive disorder
- recurrent lung infections-pseudomonas, failure to thrive

- MC bug: Streptococcus pneumoniae
- Hallmark: fever cough tachypnea
