Malignant gliomas and brain metastases often lead to death in months

Benign: more
- Usually encapsulated or isolated from surrounding
- Grow slower spread less frequently
Malignant: less
Most common primary brain tumor?
Gliomas, including:
- astrocytomas - most malignant = GBM
- ependymomas - 4th ventricle in kids
- medulloblastomas
- oligodendrogliomas - frontal lobe in adults
Metastatic or primary brain tumors more common?
Mets
Typical CT appearance Metastatic brain tumors?
Multiple, spherical tumors at gray-white junction of cerebral cortex, with surrounding edema
Most common brain mets?
- Lung
- Breast
- Colon
- Kidney
- Melanoma
When are Primary brain lymphomas seen?
AIDS
Use steroids in brain tumors?
May decrease edema around tumors, and reduce neurological symptoms
What is Meningeal carcinomatosis?
Syndrome where cancer cells spread throughout CSF, depositing over different areas of cortex, cranial nerves or spinal nerve roots
- Diagnosis suggested by AMS, HA, CN palsies or radiculopathies
Most common benign brain tumor?
Meningiomas - best prognosis all brain tumors when excision possible
Tumor causing hormonal signs?
Adenomas
Rx adenoma?
Transsphenoidal approach through skull base
Most common spinal cord tumors?
- Meningiomas
