patient safety- define
freedom for patients from unnecessary or potential harm arising from healthcare
Patient safety incident
unintended or unexpected incident which could have or did lead to harm for one or more patients receiving NHS funded care
human error
occurs when actions and decisions of individuals result in failures that can immediately or directly impact patient safety
harm
injury, suffering, disability or death: it can be physical, psychological, financial, reputational
risk in healthcare
likelihood of harm that somebody or something will be harmed by a hazard multiplied by severity of potential harm
Levels of harm:
no low- first aid only moderate- increased care required severe- permanent death
victims of harm
first: patient, physical/ psychological
second: staff, relatives, usually psychological
third: organisation, reputational, financial
Culture vs climate
culture: norms, values and basic assumptions of an entire organisation
climate: employee’s perception of particular aspects of organisation’s culture
Active errors
Slips, lapses (skills based), mistakes (knowledge/decisions) and violations/ non- concordance
• Active errors are pushed to occur by latent/ contributory factors
