Include treatment notes and radiographs, but NOT financial records. Financial records are kept separately from the treatment records.

Correcting a chart entry
A
Entries are ALWAYS made in ink. Corrections are made by drawing a single line through the incorrect entry, initializing and dating the change.
Unlawful to obliterate an incorrect entry.

Who “owns” the dental records and radiographs?
A
The dentist “owns” all patient records, including radiographs.

A
An individual who has been examines and diagnoses by a licensed dentist and has had treatment planned by the dentist.
