Pathophysiology of angle-closure of glaucoma - PACG Flashcards
1
Q
What does PACG involve?
A
- mechanical obstruction aq humour outflow through trabecular meshwork by peripheral iris
2
Q
What are two major mechanisms of trabecular meshwork obstruction by peripheral iris
A
- pupillary block
2. iris plateau - abnormality of iris
3
Q
What is pupillary block?
A
- complete/functional apposition central irisi to anterior lens = mild dilation pupil
- trapped aq humour in posterior chamber = increase pressure behind iris
- cause peripheral iris bow forward + obstruct trabecular meshwork
4
Q
What is plateau iris?
A
- anterior displacement of peripheral iris
- caused anterioul positioned ciliary process
- peripheral iris bunches as eye dilates
5
Q
What do pupillaroy block + plateau iris cause?
A
- occlusion aq humour outflow
2. IOP elevation –> vision loss in hrs -dayss
6
Q
How is the degree of iridotrabecular contact - angle closure assesed?
A
- visulaize anatomy iridocorneal angle
- examine by gonioscopy
