IBD - presentation
females, teens-30’s
bloody diarrhea + abdominal pain
Wall involvement
UC: mucosa and submucosa ulcerations
Crohn’s: full-thickness
Location
UC: Continuous-starts at rectum, can extend as far as cecum
Crohn’s: mouth to anus w/ skip lesions
Inflammation
UC: crypt abscesses w/ neutrophils
Crohn’s: lymphoid aggregates w/ granulomas
Gross appearance
UC: “Lead pipe colon” (loss of haustra) and pseudopolyps
Crohn’s: “Cobblestone” appearance and strictures (“String sign”)
Complications
UC: Toxic megacolon and carcinoma
Crohn’s: malabsorption + nutritional issues, fistulas, carcinoma
Associations
UC: p-ANCA (+), primary sclerosing cholangitis
Crohn’s: ankylosing spondylitis, polyarthritis, erythema nodosum, uveitis
Smoking
UC: protects against
Crohn’s: increases risk
