PID etiology 2 types
Gonococcal caused by N gonnorhea
Non gonococcal caused by chlaymdia- streptococci- staphylococci.
Where does infection start in gonococcal PID and mention the pathology ?
Infection starts first at bartholins gland, endocervical gland or paraurethral gland
Tube ovarian abscess pyosalpinx, infiltration of leukocytes, healing by fibrosis causes tubal narrowing- cilia destruction and Ectopic pregnancy or infirtility
Non gonococcal infection pathogenesis/ cause ?
It is caused by induced labor, abortion, or surgical procedure.
Patho-> it spreads upwards in the uterus by the lymphatics and venous channels which cause deeper layer inflammation
Complications of PID?
Peritonitis, tubal obstruction bowel obstruction, bactremia, ectopic pregnancy and infirtility
Ectopic pregnancy causes and gross ?
Cause: PID- Peri tubal scarring- use of intrauterine contraceptive.
Gross hematosalpinx, uterine tubes has fetal parts and placental tissue
Microscopy, clinical and complications of ectopic pregnancy ?
Micro: Placental villi in uterine tubes.
Clinical: severe abdominal pain and vaginal bleeding 6-8 weeks from last menstrual cycle
Comps: rupture/haemorrhagic shock
