Carcinoid syndrome- sx
facial flushing, edema of head and neck, abdominal cramps, diarrhea, bronchospasm, cardiac lesions/ valvular disease, telangiectasias
Carcinoid syndrome- Eti
10% of patients with small bowl carcinoids. Serotonin in large quantities, amines, prostaglandins.
Carcinoid syndrome- Dx
Elevated serotonin levels.
Lambert-eaton myasthenic syndrome- Eti
Defective release of Ach in response to nerve impulse leading to weakness in proximal muscles. Small cell lung carcinoma.
Lambert-eaton myasthenic syndrome- Sx
Variable weakness, improves with activity, power increases with sustained contraction, dysautonomic symptoms present
Lambert-eaton myasthenic syndrome- Dx
Electrophysiology- motor nerve response increases remarkable
Lambert-eaton myasthenic syndrome- Tx
Predinisone, potassium channel agonists.
Hyponatrimia/ SIADH- Eti
SIADH= most common cause of hyponatremia in hospittalized patients. Abnormal H2O balance due to abnormal ADH secretion leading to excess. Carcinomas
Hyponatremia/SIADH- Sx
Neurologic & metabolic disorders- lethargy, weakness, confusion, seizures.
hyponatremia/SIADH- Dx
Decreased serum osmolulity with increased urine osmolality
hyponatremia/SIADH- Tx
Reduce serum sodium concentration
Hypercalcemia- Eti
Small lung cancer, breast, renal, adrenal, multiple myeloma & prostate . Due to effects of destruction of normal tissues by tumor- osteolytic skeletal metastases. May be more urgent in treatment than cancer
Cushing syndrome- Eti
Caused by Excessive ACTH production. Associated with lung cancer, adrenal, renal and prostate cancer.
Cushing syndrome- Sx
central obesity, moon face, buffalo hump with thin extremities.
Paraneoplastic syndrome
Aberrant tumor production of hormones, peptides or cytokines creating . Small cell lung cancer most commonly associated.
