What are the 5 symptoms of papilledema?
- HA
- Nausea/Vomiting
- Horizontal Diplopia
- TVOs
- Pulsatile Tinnitus
What are the 5 features of papilledema?
- Axoplasmic Stasis
- Obscuration of Vessels
- Paton’s lines
- Extruded Axoplasm
- Hemes
What does the ICP have to be in order to have papilledema?
greater than 200-250mmH20
How many SVP rhythmic beats must you view to r/o papilledema?
3-5 beats
What must the ICP be to observe SVP?
< 180mmH20
What are the 3 causes of pseudopapilledema?
- Obesity
- Anomalous disc (hypoplastic/drusen)
- Migraines
What are the main 3 tests to order of a patient suspected of having papilledema?
- MRI w/ contrast- to look for a mass
- MRV - to look for a blockage
- Spinal tap - to determine ICP
A (+) fluid sign on the lateral 30 degree test determines what?
Increased subarachnoid fluid, suggestive of papilledema
If a tumor is present on a patient’s MRI, is a spinal tap still needed?
No! don’t do spinal tap if tumor present
- always do imaging before lumbar puncture
What are the causes of increased ICP?
- Obstruction of Flow (i.e. mass)
- Blockage of venous sinus drainage
- Elevated intra-abdominal pressure
In order for a patient to be dx with Idiopathic Intracranial HTN, they have to meet all of Dandy’s Criteria. What are the 5 criterion?
- Pt must be awake and alert
- Must have SxS of increased ICP
- No neurological signs except CN VI paresis
- Normal imaging
- CSF pressure > 200-250 mmH20
What is the tx for IIH?
- Weight loss (10% of body weight)
- CAIs (Diamox)(1000mg/day)
- Sx tx
For a lumbar puncture, which area do they go in between?
Between L3 and L4
What is the proven cause of IIH?
Obesity
What is the likely cause of IIH?
- Chlordecone (Kepone)
