Stages of Family Illness Trajectory
Stage I: ONSET of illness to Dx
Stage II: Impact phase - RXN to Dx
Stage III: Major THERAPEUTIC efforts
Stage IV: Recovery phase: Early ADJUSTMENT to outcome
Stage V: Adjustment to PERMANENCY of outcome
Stage with the ff physician responsibilities:
- explore the explanatory model and fear that patients bring
- explore conflict of the patient and aspects of pre-diagnostic phase of family
Stage 1: Onset of Illness

- help cope by family conference, etc
- help families maintain openness to each other
- know that guilt is a natural response
- offer alternative interpretation of proposed therapeutics to bolster denial of family
Stage 2: Impact Phase

- Remain open to the family and provide info
- work in harmony w/ the fam
- Coordinate all aspects of therapy
- Anticipate pathologic response (behavioral problem, acting out)
Stage 3: Major therapeutic efforts

- Deal with immediate effects of trauma
- Alleviate anxiety and assure adeq rest
- Repeated reassurance
- Call on other members for support
- Clarify labeling once well and has returned to health
Stage 4: Early adjustment to the permanency of outcome

- Assist patient and familt in relating to health care system
- Aid them in efficient and functional readjustment
- Aid in home care

How to evaluate family in crisis?
- Assess FH of coping w/ problem & stressor
- Determine style of family devt
- Check role of pt in family
- Monitor role disruption
