stage 1-4
1 is asymptomatic
4 is ulceration and gangrene
what stages are critical limb ischaemia?
3 and 4
3 = rest pain
4 = ulceration and gangrene
symptoms of critical limb ischaemia ?
pain at rest
burning pain at night which is relieved by hanging legs over the side of the bed
arterial ulcers ?
punched out ulcer, usually small and circular
found on lateral side of the ankle
intensely painful
signs
diminished or absent pulses
dry, waxy, cold skin with hair loss +- ulcers
what ABPI should you use no compression?
less than 0.4
5ps of an acutely ischaemic limb?
pale pulseless pain paralsysis paraesthesia perishingly cold
how is acute limb ischaemia managed?
as a surgical emergency
if diagnosis is in doubt - angiography
if not - proceed to management
embolectomy, thrombolysis, revascularisation, amputation
tapering of legs above ankles, resembles upside down champagne bottle
lipodermatoscleroris
what is lipodermatosclerosis a sign of?
chronic venous insufficiency
due to adipose inflammation and causes legs to become swollen and skin hardened
management of varicose veins?
avoid prolonged standing, weight loss, exercise
surgical management of varicose veins?
injection sclerotherapy or removal of the veins
scoring system DVT ?
wells score
> 3 points is considered?
very high probability
wells score 1-2 is consideere?
intermediate risk
