Warfarin: efficacy
Measure initial INR after first 2-3 doses & subsequently at intervals no longer than every 4 wk
once stable dose achieved, may monitor 12 wk in stable pt
Pt at high risk of bleeding require more frequent monitoring
INR target and therapeutic range depend on indication: AFib/AFlutter target: 2.5 (range 2-3), prosthetic heart valves: 2.5 (range 2-3), mechanical mitral or aortic valve: 3 (range 2.5-3.5), MI & ST elevation: 3 (range 2.5-3.5 w/ aspirin), venous thromboembolism, prophylaxis, & tx (including pulmonary embolism, DVT, hip/knee arthroplasty: 2.5 (range 2-3).
