Q

A 72-year-old woman with ovarian cancer is seen in the gynaecological oncology clinic. There, the consultant talks through her pre-surgical prognosis, based on her risk malignancy index (RMI). What are the three components of the RMI?

A

CA-125 levels

Menopause status

USS findings

Q

Standard treatment for someone with ovarian cancer?

A

Staging laparotomy:

  • Midline laparotomy
  • Hysterectomy
  • Bilateral salpingo-oophrectomy
  • Omenectomy
  • Lymph node sampling (para-aortic/pelvic nodes)
  • Peritoneal washing (saline solution injected into peritoneal cavity and then removed for cytology)

16
Q

How to manage confirmed Stage 1 ovarian cancer?

A

Perform retroperitoneal lymph node assessment as part of optimal surgical staging

17
Q

What sort of chemo used for the different stages of ovarian cancer?

A

Stage 1c/grade 3 - 6 cycles of carboplatin

Stages 2 - 4: paclitaxel

Do not offer chemotherapy to women who have had optimal surgical staging and have low-risk stage I disease (grade 1 or 2, stage Ia or Ib).

18
Q

What is the USS appearance of ovarian cancer?

A

Septations

Echogenic mass (Solid area)

Bilateral

Ascites

Multiloculated appearance

Increased blood flow
