Ovarian Cancer Flashcards
What are the four histological types and what is the tissue of origin?
High-grade serous - Ovaries and fallopian tube
Clear cell - uterus
Endometrioid - uterus
Mucinous invasive - GIT
When is BRCA testing indicated?
In all women diagnosed aged <70 with high frade non-mucinous epithelial ovarian, fallopian tube, or primary peritoneal cancer
At what stage to patients usually present?
Stage III/IV
What is the primary treatment of Stage III/IV?
Debulking surgery
Primary chemotherapy
What is the primary chemo regimen for stage III/IV?
6 cycles of carbo-paclitaxel, every three weeks
What is the most important prognostic factors in the setting of disease recurrence?
Platinum sensitivity
When is neoadjuvant chemo indicated?
As a bridge to surgery in the setting of severe symptoms at presentation
What is the limiting factors for intraperitoneal chemotherapy?
Severe side effects
What pathophysiological mechanism is targeted with novel therapies?
Angiogenesis
Which biologic is used to target angiogenesis?
Bevacizumab
When is bevacizumab helpful in first line therapy?
When surgical debulking is suboptimal
When are PARP inhibitors useful?
In patients with BRCA mutations for maintenance
Which PARP inhibitor is used?
Olaparib
