Presentation of ovarian cancer
silent killer, early fullness, bloating, n/v, pain
possible pleural effustion
Highly elevated CA125
system used for staging in ovarian cancer
FIGO
Chemo treatment for stage 1C high grade?
Platinum/taxane
IV only
Chemo treatment for stage 2 and 3
Platinum/taxane
IV or IP
Chemo treatment for stage 4
Platinum (single)
IV only
Who gets IP therapy
Stage 3 after surgery into peritoneal cavity
Do not warm fluid
NS then chemo then NS
reposition every 15 minutes
Platinum agents used in endometrial cancer
Cisplatin
Carboplatin (give growth factor with, dose by AUC)
cause nausea and vomiting, neurotox
Delayed hypersens after 6-8 cycles
Taxane agents used in endometrial cancer
Paclitaxel Docetaxel Cause alopecia and neuropathy Infusion reactions from Cremophor in formulation (more in pac) Premed with antihistamines and steroids
VGEF inh for ovarian cancer
bevacizumab
during treatment then maintanence
When to use a single platinum
Poor performance status Comorbities Stage 4 disease >65 years old If unable to tolerate IP or combo IV
