Rocky Mountain Spotted Fever prevalence & location
In pretty much every state in the U.S.
- most frequent in southern appalachia
RMSF bactieria
Rickettsia rickettsi
RMSF clinical presentation
- abrupt onset of flu like symptoms
- small red macular rash on palms and soles
RMSF tests
- Liver function test
- bacteria initially travel in blood and present on skin
RMSF Treatment
Doxycycline
What type of parasite causes Babesiosis?
- hemato-parasites (like malaria) that infect RBCs
- B. microti
Babesiosis epidemiology
- carried by “deer tick” same species’ as lymes
- can get it from a blood transfusion because there are no required screenings for babesiosis parasites
Babesiosis clinical presentation
- FLU LIKE SYMPTOMS IN THE SUMMER
- wrong time of year
- gradual onset of illness
- no diagnostic rash
Babesiosis tests
- blood smear (gimpsa smear stain)
- get purple rings in the RBCs from trophozoites
Babesiosis treatments
- Quinine + Clindamycin (14 days)
- DOXYCYCLINE IS NOT ACTIVE
- chloroquine reduces symptoms but doesnt kill parasite
Tularemia (aka Rabbit Fever) epidemiolgy
- infection via inhalation, fresh blood, or flesh
- vectors: ticks, rabbits, deer flies
- mostly in south central U.S.
Tularemia clinical presentation
- Abrupt onset of flu like symptoms (fever/chills)
- NON-HEALING ULCERATED WOUND COVERED BY BLACK ESCHAR (SCAB) at wound site
Tularemia lab tests
- F. tularensis is biosafety level 2 rated
- WBC & LFT may be elevated
- AB’s last for along time (like lymes disease)
Tularemia treatment
- Doxycycline (14 days)
- common to give doxy + ciprofloxacin
