Osteoporosis Flashcards
T score range in osteopenia?
-1 - -2.5
T score for osteoporosis
> -2.5
Name the 2 type of osteoporosis?
- Postmenopausal
2. Senile
What is the age for senile OP?
> 75
What is secondary OP?
Steroid induced
What is a T score?
BMD of young healthy patient of same sex and race
What is a Z score
BMD of aged matched population of same sex and race
Fragility fractures in order of occurrence?
- Vertebral body
- Hip
- Wrist
What risk assessment tool is used for OP?
FRAX score = risk of OP # in next 10 years
Indications for treatment in OP? 3
- Post menopausal female/ Male >50 with hip/vertebral fracture
- T score of -1.5 - -2.5 with RFs
- T score of >-2.5
With a t score of -1.5 to -2.5 when would you consider therapy?
- 10 year hip fracture risk >=3% (HIP 123)
2. FRAX >20%
What is the Singh index?
Changes in the trabecular pattern of the proximal femur as index of OP
What is ward triangle?
Weak point of femoral neck at junction of three trabecular groups
What are 2 prophylactic agents used?
- Vit D - 800-1000iu/day
2. Calcium - 1000-1200mg/day
What are the groups of treatment options available for OP?
- Bisphosphonates
- Hormones
- Immunoglobulins
What are the 2 types of bisphosphonates?
Nitrogen and non nitrogen
What is the mechanism of action of the bisphosphate group?
Incorporated into bone
Released when OC reabsorb bone
Inhibit OC - Apoptosis or Flatten ruffled border
What is the mechanism of action of non-nitrogen bisphosphonates?
Induce premature OC apoptosis
What is the mechanism of action of nitrogen bisphosphonates?
Inhibit Farnesyl Pyrophosphate synthase (Mevalonate pathway)
Major side effects of BP therapy?
- ON of the jaw
- Atypical subtrochanteric femur fractures
- GI symptoms
Examples of NONnitrogen containing BP?
Clodronate
Tiludronate
Etidronate
CLOTILdE
What are the hormone treatments available?
- HRT
- ERT - no uterus
- Calcitonin
- Raloxifene
- Testosterone (not approved)
- PTH - Terliparatide
What is Raloxifene?
SERM - Selective oestrogen receptor modulator
When may you not use telipratide?
Pagets disease = increased Osteosarc risk
What are the 2 immunoglobulin therapies available?
Denosumab
Romosuzumab
What does Denosumab do?
Monoclonal Ab against RANKL (Similar to osteoprotegrin)
