what is osteoarthritis?

is bone normally in contact with other bone?
cartilage always makes contact with other cartilage and synovial fluid reduces the friction but osteoarthritis damages cartilage between the bone so touches bone
which joints are affected first?
wt bearing joints
is this inflammatory damage?
usually non inflame damage bc inflam is not what causes the damage (some) but not the primary damaging process
is this a slow or fast progress?
slow but a kid can get it if he injuries a wt bearing joint with continuous motion of joints (Secondary to injury)
Et ?
primary is idiopathic wear and tear with ageing genetic predisposition genes that help with repair and maintenance with cartilage might be defected easier cartilage damage and poor repair
2ndary injury (athlete ex), obesity (bc joints are heavily burdened with mass, metabolic link in terms of the joints)
what maintains cartilage?
chondrocytes maintain cartilage
articular cartiage (covers bone in joints)
-smooth, wt bearing
-dissipates force to bone
comp & properties of cartilage change
patho?
composition and properties of cartilage change and chondrocytes release cytokines and proteases are released that are destructive enzymes that damage the cartilage and cause destruction of cartilage
what else does the chrondrocyte damage do?
impaired the ability to heal the cartilage.
why does sclerosis of bone occur? what is this?
sclerosis is hardening of bone n attempt to protect it
cartilage deteriorates and the bone is unprotected and sclerosis of bone occurs
what else appears as fluid enters cracks in bone?

cysts and fissures
osteophytes form (precipitation of bone) and joint enlarges and deforrms
what happens when force is applied to the joint?
the cartilage and bone deform (not damage but change in shape of cartilage)
normally force is distributed evenly but cartilage is not able to flatten and only at two spots will have force
manifestations of osteoarthritis?
initial: non localized aching pain
later: activity related et bearing joint pain bc joint might be in contact w bone
crepitus
stiff, inflm joints (stiff in the morning bc not using it, but hurts when you are working it)
musculoskeletal pain bc with muscles
Dx of osteoarthritis?
hx and px
xray
labs- done to exclude other arthritis
