  • Patient concerns/ questions
  • Action taken already?
  • Other medical conditions
  • Medications
  • Allergies - type/ severity
  • OTC/ herbal/ vitamins?
  • Creams/ patches/ ointments/ gels/ inhalers/ eye drops/ ear drops
  • Check relevant lifestyle behaviours
  • Clearly explain suspected diagnosis
  • Offer treatment suggestions
    ø ask if patient has preferred formulation, e.g. tabs/ solution
