Orthodontic Patients Flashcards
age
usually pre-teen/teen years when gingivitis is at a greater risk {but older pts with ortho, too}
aging can cause teeth
to drift mesially
gingivitis due to
plaque retention around appliances {can be a deposit retentive factor}
gingivitis can be
slight to severe w/ enlargement, especially around papillae
position
severe malocclusions are more susceptible to plaque retention
problems w/ appliances
accidents may cause
wires to bend and become embedded in gingiva
loosened bands
may be forced under gingiva
removable appliances or their clasps
may press against gingiva
rubber bands
may slip under gingiva and detach JE
self care is difficult b/c
appliances are in the way and interfere w/ toothbrushing and interdental devices
toothbrushing
soft brush w/ end rounded filaments; can use power assisted brush
bi-level ortho brush
can be used w/ short horizontal strokes around brackets
sulcular brushing
for periodontal tissues
charter’s method
for cleaning around bracket for gingival side
stillman’s method
for occlusal side of bracket
interdental aids
interdental tip toothpick in holder floss threader tufted dental floss interdental brush single-tuft brush oral irrigators
white spots
demineralized lesions often result from ortho therapy b/c of compromised OH during Tx
remineralized area
harder and stronger fluorapatite crystalline structure within enamel that can better withstand bacteria acid attacks
care of removable appliances
clean appliance after each meal and before bedtime
brush and rinse teeth under appliance every time appliance removed
brush mucosa under appliance w/ long, straight strokes from posterior to anterior
self-applied fluoride
mandatory, along w/ fluoride dentifrice {use gelKam in addition}
can use daily mouthrinse, gel tray, or brush on gel
usually prescribed stannous fluoride (GelKam or prevident)
space maintainers can be either
fixed or removable
space maintainers can use any
of the methods previously described for plaque control; single-tuft brushes w/ bent shank may be helpful
space maintainers; adaption of methods
needed for cleaning lingual arch wire and lingual aspect of molar bands or fixed appliance
space maintainers; removable space maintainers require
a clasp brush and separate toothbrush for cleaning can use denture brush
