OG/NG tube Flashcards
what is the purpose of nasogastric tubes
1. Allow drainage of the contents of the stomach – Decompress stomach for ventilation – Gastric lavage (overdose) – Bleeding – Air insufflation removal
2.Provide a safe access route to the
gastrointestinal tract for the safe administration
of fluids, drugs and / or nutrients.
- Reduce/stop life threatening bleeding due to
esophageal or stomach varices when other
methods are unavailable/unsuccessful - Measure pressure in the esophagus for
transpulmonary pressure monitoring - Provide access to phrenic nerve for neurally
adjusted ventilatory assist (NAVA)
contraindication of OG/NG tube
Severe facial/cranial trauma
What are 2 types of NG tube used?
Levin
Salem Sump
fxn of levin
for Gastro
intestinal drainage
fxn of salem sump/irrigation tube
used for irrigation and tube feeding
What is esophagogastric tamponade
bleeding varices of the esophagus and stomach
how to treat esophagogastric tamponade
Sengstaken-Blakemore tube
a balloon is inflated within the esophagus and stomach to apply pressure on bleeding blood vessels, compress the vessels, and stop the bleeding.
What are the ports of the Sengstaken-Blakemore tube
- gastric aspiration
- balloon inflation
- esophgeal balloon inflation
How to measure NG length
– From mouth to ear
– From ear to xyphoid
– Place tape at this spot on tube
complication of inserting NG
• Gagging/vomiting
– Ensure oral suction available
– Chill tube
– Use topical anaesthetic
- Aspiration
- -> patient sit 30o or higher
• Tissue damage
how to check for tube placement
Aspirate if no, insert 10 - 20 ml aspirate if no, advance tube 5 - 10 cm aspirate if no, C-xray
if aspirate is obtained, then check PH
Ph 6-7, correct placement
ph 7-8, too far in
how to insert a esophgeal ballon catheter
inserted to a depth of 60 cm
Gentle compression of the abdomen is
performed to confirm placement
The catheter is then pulled back 40 cm
Cardiac oscillations should be present if
placement is correct
