How long must obsessions/compulsions be present to make the diagnosis?
Most days for two weeks
A rating scale which may help assess the impact on the patients life
Yale-Brown Obsessive Compulsive scale
First-line treatment in mild functional impairment
Low intensity psychological treatment e.g. CBT, plus exposure and response prevention
if unable: SSRI
Treatment for moderate impairment
High intensity CBT + ERP, or SSRI/clomipramine
What does ERP involve?
patients are repeatedly exposed to the situation causing them anxiety and prevented from performing repetitive actions

Fear of harm (e.g. locked doors)

Overestimation of threat/inflated sense of responsibility;
Beliefs about the important of, and need to control, thoughts
Perfectionism and intolerance of uncertainty
Five screening questions for OCD
- Do you wash or clean a lot?
- Do you check things a lot?
- Is there any thought that keeps bothering you, that you would like to get rid of but can’t?
- Do your daily activities take a long time to finish?
- Are you concerned about orderliness or symmetry?
