Under the terms of the Mental Health Act, what is any registered medical practitioner able to do?
Perform an emergency detention (usually with consent of a mental health officer, but can proceed without)
How long does emergency detention last for?
maximum 72 hours
What terms must be met for a short term detention to be granted? (5)
- Patient has a mental disorder
- Their ability to make decisions about care is likely impaired
- Medical treatment is available
- They are at risk of causing harm to themselves or others
- Granting the order is necessary
How long can a compulsory treatment order last for?
Up to 6 months
What is the first-line option for tranquilising patients?
Try non drug approaches e.g. talking, seclusion, distraction
What is the first-line drug option in tranquilisation?
Oral lorazapem 1-2mg
If patient has history of significant atypical antipsychotic therapy- consider adding haloperidol 5mg
When should intramuscular injection be considered?
Oral is unsuccessful, or effect required within 30 minutes
What formulation should the intramuscular injection be?
Lorazepam 1-2mg (Mixed 1:1 in water for injection or 0.9% sodium chloride)
How should management with antipsychotic drugs be initiated? (2)
Lowest possible dose, 2 week trial
First-line antipsychotics
Atypicals e.g. risperidone
How should treatment-resistant illness be managed?
Clozapine
What does the mental health act specifically preclude from being grounds for compulsory detention?
Under the influence of drugs/alcohol
What happens after the 6 months of a compulsory treatment order expires?
Re-apply for another 6 months, then 12 months
Can be appealed and taken to tribunal
