When is IOL indicated
A
Prolonged preg- 41-42 weeks (if refuse- 2x/w USS and CTG)
Maternal request in exception circumstances (partner leaving soon etc)
Stillbirth
any disease with increased risk of stillbirth
PET, GDM, obstectric cholestatsis
can be used in PPROM after 24h if not happening and post 34w

When should IOL be avoided
A
Avoid in PPROM before 34w BREECH- avoid IUGR- avoid, do Csec if compromise Macrosomia - AVOID VBAC- avoid

Methods of IOL
A
Membrane sweep -adjunct but should done before all others
dilation methods-- Prostaglandins-preffered method Pessary - 1 dose in 24h Gel - 1 dose last 6h, give max 2 Baloon dilation
